Nose being the main element of facial aesthetics and with the functions of great importance both respiratory and olfactory, we must treat it fully to achieve better functional and aesthetic result.
It is a surgical technique that allows visualization of the nasal anatomy so that we can predict the results and obtaining a higher septal approach. It is vital careful excision of tissues and restoration of the supporting structures.
The purpose of this paper is to describe key points of technique that can help us achieve better results and prevent complications.
We emphasize the questioning, especially the background. Included are functional exploration to determine if needed in the special paraclinical sinus CT. We take digital patient in different projections and talked to him about the achievements that as surgeons can perform in any particular case to avoid creating false expectations.
We have concluded through these years:

4) The aesthetic results are predictable in the short term.
5) nasal function is compromised.
Disadvantages: may be the prolongation of surgical time (2 hours) and half columellar scar.
